Encinitas, CA – March 26, 2018  – Aging Life Care Association™ (ALCA) has approved Lauren Reynolds, CHCM and Founder/Administrator of At Home Nursing Care, as a professional member.  By passing the review process to become a member of the Aging Life Care Association, Ms. Reynolds adheres to the Standards of Practice and Code of Ethics developed by the association and its members to ensure public accountability.

A care manager creates a holistic plan to help clients that is based on the client’s needs, values and goals.  It’s a client-centered or patient-centered plan of care which honors individual choice while working with family concerns and also minimizing safety concerns.    As the Administrator of At Home Nursing Care, Reynolds is charged with supervising her RN Case Managers and reviewing plans of care at regular intervals to make sure they are complete, based on client’s needs, values and goals and also promote safety and independence, as appropriate. 

Reynolds has worked as an Aging Life Care Professional™ (also known as a geriatric care manager) since May of 2010.  She joined the field of elder care after retiring from a 15 year career as a News Reporter/Anchor for ABC 10News in San Diego.  Her mother’s battle with terminal breast cancer inspired Lauren to open her agency.  She has been active in Rotary as the volunteer Chair for the Taste of Rancho Santa Fe,  She has been the Co-Chair of the Country Friends Giving Hearts Gala and the founder of the At Home Nursing Care Foundation. She’s also on the Board of the California Association for Health Services at home (CAHSAH).

Reynolds provides guidance and advocacy for families who are caring for older relatives or disabled adults. With a specialized focus on issues related to aging, she assists clients with challenges such as creating a safe home environment, in-home care, referrals to medical providers or elder law attorneys, as well as advocacy and support.
